The Emergency Food Assistance Program (TEFAP)

The USDA & New York State now require the food pantry to obtain an attestation form.  The form is used to prove your eligibility to receive assistance from TEFAP.  You will be asked to complete the form at the pantry.  The form can be found here TEFAP Attestation with a Spanish version here TEFAP Attestation - Spanish.
